Form Instructions

Your family history information must date back to 1901.
Research is done for families that lived in the Saskatchewan, Assiniboine and Red River Valleys. Check our map to see if your family lived in these areas.
Our genealogy is based on Metis Scrip applications. To learn more about Metis Scrip, and to check to see if you feel your family would have applied for this scrip please see our page on Metis Scrip records.
All fields denoted with an asterix (*) must be completed for form to send
Start at the top of the form and work your way down
Fill in as many of the fields as possible
If a "Parents of:" field is blank, move down to the next "Parents of:" field
Additional information may be submitted in the last field, at the end of the form
Check over your information before completing!
Wait for the online form to be e-mailed to your address (this should take less than five minutes).
